Requesting Your Medical Records
For patients who need to obtain a copy of their medical records from Providence Family Medicine, we have a straightforward process. To initiate a medical record request, you will need to complete and sign a Release of Information form. Please ensure you clearly indicate the specific medical information you are requesting on the form.
Please allow up to 30 days for the processing of your medical records request. Our Medical Records department will contact you as soon as your records are ready for collection. It’s also important to note that your doctor’s office can request copies of your medical records on your behalf at no charge, which may be a quicker option in some cases.
