Your Primary Care Physician and Care Team Approach
At CNY Family Care in Brittonfield, we prioritize continuous and comprehensive healthcare for every patient. To facilitate this model of care, we ask each patient to select a personal Primary Care Physician. This physician will be your main point of contact, establishing a lasting relationship with you and overseeing your healthcare journey. Your chosen Primary Care Physician leads a dedicated care team comprised of nurses, Nurse Practitioners, Physician Assistants, and Diabetic Educators. This team works together to ensure you receive appropriate age-related preventative screenings and treatments, along with the most current, evidence-based treatments for any individual health conditions you may have.
Important Information Regarding New Patients
Please note that CNY Family Care does not accept Workers Compensation Insurance. Currently, we are focused on welcoming new infant, adolescent, and young adult patients who are transitioning from pediatric care. At this time, we are not accepting new adult patients.
