We understand that unforeseen circumstances can arise, making it necessary to reschedule or cancel appointments. We kindly ask for a minimum of 24-hour notice if you need to change your appointment. When you arrive at the Emergency Department, please inform them of your primary care provider’s name at Maplewood Family Practice. This ensures seamless communication and allows the Emergency Department to transmit all relevant details from your visit directly to your primary care physician’s office, facilitating coordinated follow-up care.
Understanding Fees, Insurance, and Financial Assistance
Maplewood Family Practice is a preferred provider with the majority of insurance companies. To confirm whether we are in-network with your specific insurance plan, we recommend contacting your insurance provider prior to your appointment. Please remember to bring your insurance card with you to each appointment. As a service to our patients, we will bill your insurance company directly. Co-payments mandated by your insurance plan are due at the time of service. For your convenience, we accept payments in the form of cash, checks, MasterCard, and Visa.
