It is important to carefully read all instructions provided with the application before you begin filling it out. If you choose to apply by mail or fax, make sure you sign and date the form before submitting.

To process your Temporary Disability Insurance application efficiently, the State of New Jersey requires information from both you and your medical provider. It is your responsibility to ensure that all necessary information, including the medical certification from your doctor, is submitted through the online system, by mail, or by fax.

Here’s a breakdown of the information required for your application:

  1. Information You Need to Provide:

    • Your Social Security number, current contact details, and date of birth are essential for identification.
    • Clearly state the exact date you became unable to work due to your condition.
    • Provide complete contact information for the medical provider who treated you within 10 days of your disability onset.
    • If applicable, include dates of any emergency room visits, urgent care treatments, or hospitalizations related to your condition.
    • Detail your employment history for the last 18 months, including contact information for all employers and the work addresses.
    • Specify any paid time off or other benefits you have received from your employer since your last day of work.
    • Indicate the expected date of your recovery and return to work, or the actual date if you have already returned.
  2. Medical Certification from Your Doctor:

    • Your medical provider must submit a medical certification. This certification is crucial as it confirms the period you are medically unable to work. You can find a list of approved medical practitioners on the NJ Department of Labor website.

When you complete your portion of the application online, you will receive a unique Online Form ID. It is vital to provide this ID to your medical provider. They will use this number to submit their medical certification online, directly to the New Jersey Department of Labor. After completing your part, you will be prompted to print or save a PDF containing your Online Form ID and instructions for your medical provider. If you cannot print immediately, you can access and print these instructions within 14 days by clicking on Access claim documents at the top of the application page.

To keep your draft application active, remember these timelines: (1) within 14 days of starting, complete your information and confirm your claim, and (2) within 14 days after your leave begins, certify and officially file your claim.

If you are applying after your leave has already started, you have a 30-day window from the first day of your leave to submit your application for Temporary Disability Insurance under NJ state family leave.
