Orchard Park Family Practice P.C. is proud to announce its highest level recognition from the National Committee for Quality Assurance (NCQA) for Physician Practice Connections-Patient Centered Medical Home. This prestigious acknowledgment highlights Orchard Park Family Practice’s commitment to providing exceptional, patient-focused healthcare.
The Patient Centered Medical Home (PCMH) model emphasizes a partnership between patients, their personal physicians, and when appropriate, their families and other healthcare specialists. Orchard Park Family Practice utilizes this model to ensure patients receive coordinated and comprehensive care. By leveraging registries, health information technology, and health information exchange, the practice ensures that patients receive the right care, at the right time, and in the most appropriate setting. This patient-centric approach is delivered in a manner that is both culturally and linguistically sensitive to meet the diverse needs of the Orchard Park community.
Embracing the PCMH concept allows Orchard Park Family Practice to move beyond simply treating illnesses or injuries as they arise. Instead, the practice focuses on building long-term, care-coordinated, and preventative relationships with patients. This proactive approach fosters a sense of trust and security, assuring patients that Orchard Park Family Practice is deeply invested in their ongoing health and well-being. The NCQA recognition further validates the practice’s dedication to continuous improvement and patient care excellence.
