Carolina East Family Medicine Announces Service Discontinuation: Information for Patients

Dear Patient,

We are writing to inform you about the difficult decision to discontinue medical services at Carolina East Family Medicine, effective September 3, 2024. This was not an easy decision, and we understand it may cause disruption in your healthcare. We sincerely regret that we will no longer be able to serve as your primary care provider.

To ensure a smooth transition, we encourage you to begin looking for a new physician to continue your medical care. For assistance in finding a qualified healthcare provider in your area, we recommend reaching out to the North Carolina Medical Society. They can provide resources and referrals to help you find a new family medicine practice that meets your needs. Additionally, your insurance carrier can offer a list of in-network providers near you.

Accessing Your Medical Records:

Your medical records are important, and we want to make sure you can easily obtain them. To request your records, please send an email to [email protected]. For us to comply with HIPAA regulations and release your information, you will need to complete and sign a medical release form. This form is available on the Quasar Health Solutions website at https://quasarhealthsolutions.com. Please submit your medical records request within 30 days of our closing date, specifically by October 3, 2024.

Medication Information:

We want to support you in maintaining your medication regimen during this transition. If you have been seen at Carolina East Family Medicine within the last six months and are on maintenance medications (such as for diabetes, high blood pressure, or high cholesterol), we will provide a 90-day supply. For patients on controlled medications, within legal limits, we will provide a 30-day supply. These prescriptions will be sent to the pharmacy we have on file for you. If you are prescribed controlled medications, it is particularly important to find a new primary care provider as soon as possible. We will be unable to provide refills for controlled medications beyond this 30-day period, making it crucial to establish care with a new physician without delay.
