Continuous and Comprehensive Family Health Care
Our primary goal at CNY Family Care is to offer continuous and comprehensive healthcare for all our patients. To facilitate this model of care, we ask each patient to select a personal Primary Care Physician (PCP) within our practice. Your chosen physician will serve as your main point of contact, building an ongoing relationship with you and overseeing your healthcare journey. This PCP leads a dedicated care team, consisting of nurses, Nurse Practitioners, Physician Assistants, and Diabetic Educators, who will work together to ensure you receive coordinated and personalized care during your visits. Our team is committed to providing age-appropriate preventative care, screenings, and treatments, along with the most current evidence-based medical interventions for your specific health needs.
Welcoming New Infant, Adolescent, and Young Adult Patients
Please note that CNY Family Care, LLP does not accept Workers’ Compensation Insurance. Currently, we are excited to welcome new patients who are infants, adolescents, and young adults transitioning from pediatric care. While we are not accepting new adult patients at this time, we encourage families with younger members to explore the comprehensive and compassionate care offered at CNY Family Care.
