Please note that our phone lines are managed by an answering service during lunch hours, from 12:00 PM to 1:00 PM. Additionally, our answering service may handle calls during the first and last hour of our business day, even while the office is open for patient appointments. Our office observes closures for most major holidays.

Urgent Care, After Hours, and Emergencies
For medical issues that arise outside of our regular office hours, please call our main number at 508-541-2436. Our on-call physician will promptly return your call after you leave a message. In the event of a life-threatening emergency, please dial 911 immediately for emergency medical services.
Referrals to Specialists
Occasionally, your Franklin Family Practice physician may recommend a consultation with a specialist. If your insurance plan necessitates a referral from your primary care physician, kindly contact our office during our regular business hours at least seven days prior to your scheduled specialist appointment. This advance notice allows us ample time to process your referral efficiently.
Medical Records Access
To obtain a copy of your medical records, please download our Medical Record Release Form. After printing and completing the form, please return it to Milford Regional Physician Group via mail or fax, using the details provided on the form.
Insurance and Payment Options
Franklin Family Practice participates with the majority of medical insurance plans. Given the wide variety of insurance options available, we recommend contacting your insurance provider directly to confirm that your specific plan is accepted by our physicians. We are committed to providing equitable access to care for all patients, regardless of their payment method. If you have insurance coverage, please remember to bring your insurance card with you to each appointment, as we are required to verify your insurance at every visit. Co-payments are expected at the time of service. For your convenience, we accept checks, Mastercard, Visa, Discover, and American Express.
