What to Bring to Your First Appointment
To help us provide you with the best possible care during your initial visit, please bring the following items:

- A valid photo ID for identification purposes.

- A comprehensive list of all medications you are currently taking, including dosages and frequency. Alternatively, you may bring your medications with you.

Registration Process for New Patients
Please arrive at least 15 minutes prior to your scheduled appointment time to allow for completion of the registration process. You will be asked to sign our HIPAA form, as mandated by law, to acknowledge that you have been informed of our privacy practices, which can be reviewed in detail at our privacy policies.
Payment Policies
We kindly request that you be prepared to pay any applicable deductibles, co-pays, co-insurance, or outstanding balances at the time of your visit. We value your appointment time; therefore, if you need to reschedule or cancel, please notify us at least 24 hours in advance.
